During morning observations, the client appeared to remain focused during his morning reading time and ignored distractions. During midday observations, the client was observed interacting with his peers in a positive manner while handing out nametags for lunch. During afternoon observations, the client exhibited difficulty with following directions on the first prompt from his teacher. QMHP-C greeted client upon arrival to the school setting. During morning observations, QMHP-C provided the client with verbal praise and encouraged him to continue to remain focused and ignore distractions while in class. QMHP-C provided him with positive reinforcement and prompted him to continue to interact with his peers in a positive manner. During afternoon observations, QMHP-C redirected The client and reiterated the importance of following directions on the first prompt from his teacher. The client responded well towards Therapeutic Day Treatment services as evidenced by positive engagement throughout the day. The client agreed to continue to remain focused and ignore distractions. During midday he continued to interact with his peers in a positive manner. During afternoon observations, The client was receptive to redirection and agreed to follow directions on the first prompt from his teacher. The client agreed to continue to track behavior on his self-monitoring chart.
